Village Home Health and Hospice is a joint venture between Maple Knoll Communities, Inc. and Jewish Home of Cincinnati. Both non-profit organizations have a long history of serving adults dating back to 1848. Village Home Health and Hospice was established to further the mission of both of these organizations. As Greater Cincinnati’s only non-profit Home Health and Hospice organization, our primary focus is where it should be…on our patients. We serve our patients wherever they call home and take pride in providing unsurpassed care to our community.
Helping patients regain their strength and independence, Village Home Health services are designed to help one manage a chronic condition, recover from illness, injury, surgery, and remain in the comfort of their surroundings. Our dedicated team of healthcare professionals partners with every patient, their family and their physicians to ensure a coordinated continuum of care, wherever one calls home.
Patients are cared for in the setting that they consider home. Village Hospice care can be provided in the patient’s home, an assisted living facility, residential care facility, a nursing home or contracted inpatient facility. Our hospice services are delivered by a team of professionals and trained volunteers who provide physical, emotional, and spiritual care to patients, their family members and caregivers. The Village Hospice interdisciplinary team includes physicians, nurses, social workers, chaplains, State Tested Nursing Aides (STNA), bereavement counselors, and volunteers.
