Patients are cared for in the setting that they consider home. Village Hospice care can be provided in the patient’s home, an assisted living facility, residential care facility, a nursing home or contracted inpatient facility.
Hospice services are delivered by a team of professionals and trained volunteers who provide physical, emotional, and spiritual care to patients, their family members and caregivers. The Village Hospice interdisciplinary team includes physicians, nurses, social workers, chaplains, State Tested Nursing Aides (STNA), bereavement counselors, and volunteers.
We are available to help our patients 24 hours a day, 365 days a year. Our nursing team answers patient calls 24 hour a day and a hospice team member can go to the patient’s bedside day or night.
Our care extends beyond the professional services provided by the Village Hospice care staff.
We provide home delivered medical equipment, medical supplies and medications for pain and symptom management.
The Medical Orders for Life-Sustaining Treatment (MOLST) program is designed to improve the quality of care seriously ill people receive at the end of life. MOLST is based on the patient’s current health status, prognosis, and goals for care. Our MOLST trained team of professionals will walk you through the process to make sure that your final wishes for your treatment and care will be honored.
The impact of a serious illness affects the entire family. Village Hospice’s care plans and protocols focus on the needs of patients and their families. Village offers continued support during the most difficult times of loss. Our Bereavement Care team helps family members cope with grief throughout the challenging first year following a loss.
